Social Networks Clinical Survey

Dear colleagues:

Social Networks is a clinical tool designed to help support practitioners, family members and individuals with complex communication needs throughout the communication intervention process. If you (or one of your colleagues) are using it with clients and have completed Social Networks inventories (meaning you have administered the SN inventory to at least (1) one individual with complex communication needs and perhaps that individual's family member or paid work, or (2) the family member and a paid worker of an individual with complex communication needs, then we invite you to fill out the simple survey below. When you complete the survey and send it, we shall mail you a FREE package of 10 Inventory Booklets. In addition, we will welcome opportunities to support your efforts and put you in touch with others who have similar interests.

The purpose of the survey is to collect information. We plan to use the information to develop ways to better support clinicians who are using Social Networks with a variety of populations. We want to: (1) share innovative uses of the tool, (2) determine where common problems are arising, (3) disseminate information and strategies via the website and (4) hopefully, create a community of interested clinicians.

[Note: We are also preparing a section for researchers and teachers, so if you are doing research or using Social Networks in your teaching, please let me know on the survey and we'll also put you on a list to receive information from researchers and professors/teachers/workshop leaders.]

We also plan to expand our section on case examples. There is nothing more informative than reading about how someone actually used the tool and how that benefited someone with communication challenges. We have a template that will assist you in contributing a case example. We work directly with those who prepare one for use on the website and/or in other publications. Confidentiality/privacy is of utmost importance to us and will always be carefully protected.

Depending upon the detail you provide, this survey should only take between 15 and 30 minutes to complete. As we compile the information from the survey, we will use the results in development of supportive materials on the website.

Thank you for your willingness to collaborate. Please answer all the questions you can. Depending upon the detail you provide (and we welcome details), the survey will take between 15 and 30 minutes. Please don't hesitate to ask any questions you might have and/or make copies and pass this along to others you know who are using SN.

All the best,
